GOV.UK splits this into two groups and we keep its split: 6 things you’ll need to provide, and a second group it says you might be asked for depending on your circumstances. Each item carries GOV.UK’s own wording for when it applies.

Several of these come from your employer, not from you

Ask your employer for a copy of your certificate of sponsorship if you do not have one.

Four of the six required items are values your sponsor sets: the certificate of sponsorship reference number, your job title and salary as sponsored, the occupation code, and the sponsor licence number. GOV.UK's own wording for when each item applies is quoted below — you decide which of the conditional ones are yours.

Why only one item here has a date

1 of the 13 items has a validity period GOV.UK actually publishes — the tuberculosis certificate, “valid for 6 months from the date of your x-ray”. Nothing else on this list carries one, including the English language requirement. Periods that circulate for other countries are not applied here: a date we could not point at a GOV.UK sentence for would be a date we invented.
